LIABILITY WAIVER

I, {name},  acknowledge and agree that participating and/or observing in the activities within Oni Fight Team gym premises and outside of premises in my own time has inherent risks, including but not limited to:

• Injuries obtained during scheduled classes. Including but not limited to, physical trauma, joint injuries, broken bones and concussion.
• Injuries obtained within my own workout/s on gym premises. This includes but not limited to, the use of equipment and premises outside of scheduled classes
• Performing activities obtained from gym outside of premises (example: boxing combos at home), activities outside of gym scheduled with other gym members (example: participating in activities learnt from gym with other members outside of premises).
• Injuries that occur resulting from negligence or lack of adequate training.
• Injuries causing death resulting from failure or negligent misuse of the facility or any equipment of Oni Fight Team Gym.
• Injuries or death resulting from the failure of equipment and participation in scheduled classes and my own activities within Oni Fight Team Gym.

I am aware of these and numerous other inherent risks in participating and observing the activities offered and sponsored by Oni Fight Team Gym. I ASSUME COMPLETE RESPONSIBILITY and liability for those risks and for the injuries that may occur as result of these risks, even if injuries occur in a manner that is not foreseeable at the time I sign this agreement. I acknowledge that by voluntarily assuming the risks involved, I will be solely responsible for any loss or damage I sustain, including personal injuries to me, damage to my property, or damage arising out of my death.

I acknowledge that I am responsible for payments required to use the Oni Fight Team facility. Upon signing up for a membership, I take full responsibility to ensure payments are made on time. I understand that if any failed payments, late fees may occur and my membership will be frozen, with restricted access to the Oni Fight Team gym. If beyond this I am for any reason unable to make payments, I understand that Oni Fight Team admin are within their right to terminate my membership. If I require Oni Fight Team admin to freeze or cancel my membership for any reason, I understand I am responsible to request this action. I understand by doing this,  any accumulated funds on my account must be paid before this action is to occur. I understand my membership and payments will still occur even if I am not using the facility for a duration of time (holidays, illness), unless I request my membership to be frozen or cancelled. I understand this goes for private training sessions as well. I understand that any membership or personal training fees are subject to change. There are NO locked-in fees.

I understand Oni Fight Team DO NOT tolerate bullying, discrimination, sexism, racism, genderism or any form/outlet of direct or indirect harm to others. I understand that staff have the right to discontinue any membership (with reason).

I ACKNOWLEDGE that this agreement is legally binding. By signing it you give up your right to recover compensation through the courts or otherwise for any personal injury, damage to your property, or for your death, being caused by negligence, accident or otherwise.

I am aware of the potential risks and danger involved in mixed martial arts and waive and release Oni Fight Team Gym and all staff, volunteers, landlords, and owners of any liability relating to the use of Oni Fight Team Gym.
